CHADIS

CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office.

• After you complete the questionnaire, CHADIS results come directly to your doctor.
• The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.
• We use CHADIS for all of your child’s checkups, at all ages, and additionally use it for behavioral and mental health evaluations.

In order for us to provide you and your child with the best care possible, we strongly encourage you to complete the assigned questionnaires in CHADIS prior to your visit so that we may be able to address any concerns you may have during the appointment.
